If you would like to enroll your child at Action Daycare we kindly ask that you come by and visit us to request an application form. The daycare is open from 6:30AM to 6:00PM, Monday-Friday.
At this time you may also set up a date and time for a tour of our facilities.
Upon registration you will be asked to provide or fill out:
- Child's birth certificate
- Parent's birth certificate
- An allergy form
- An authorization form for administering medication
- An authorization form permitting someone other than child's parents to pick them up
- An authorization form for outings
Please take note that in regards to the admission policy, priority will be given to the following:
- Siblings of children who are already attending Action Daycare, upon availability of age group
- Children who will attend the daycare on a full-time basis (Monday through Friday)
- Children placed on the waiting list, considered according to the date of registration, and upon availability of age group
